Our proprietary intake process uncovers the root causes of your caregiving challenges, addressing the needs of your family — and importantly, you as the caregiver.
From mental health and family dynamics to financial concerns, work-life balance, and home care needs, we take a holistic approach to ensure every detail is considered.
The data we collect is integrated into our Care Planning process, designed to identify gaps in care others may overlook, provide guidance for difficult decisions, and chart a clear path forward. Personalized, responsive solutions are presented in a Care Plan, which we update as your circumstances evolve.
Your access to Family First is fully covered by Meta. Our Care Teams are here to provide ongoing support, ensuring your Care Plan evolves with changing circumstances and any new challenges that arise — so you and your family are never navigating caregiving alone.
