Our Professional Care Managers are health and human services professionals who specialize in issues relating to eldercare. As Registered Nurses and Masters level Social Workers, they are professionally trained to work with aging adults and chronically ill persons to build a stable and quality life for clients and their families.
When seniors experience illness or chronic conditions and prefer to stay home, villageplan™ is here. We provide compassionate, one-on-one care in the comfort of our client’s own home. Our personalized and affordable services are available 7 days a week and can range from as few as four hours, up to 24-hours of care each day.
While every caregiver finds themselves on their own unique life path, it can be startling to find oneself in circumstances that were not predicted or anticipated. We know that it can feel scary and lonely at times, and we understand that feelings of hopelessness and fear can arise from the unknown of the future or the known circumstances of the present.
Villageplan’s long-term care specialists are here for you! We are committed to spending the time following up with medical records, sending emails and faxes, and organizing a claim packet. We work with home care agencies, attorneys, financial advisors, assisted living communities, physician offices, hospitals, and other professionals.
